First Visit

Your initial appointment will consist of a consultation and physical exam explaining your diagnosis and treatment options. For cosmetic surgery procedures you will be given a request for your primary care physician to complete standard labs and test.

A second appointment for cosmetic (plastic surgery) and orthognathic (jaw surgery) procedures will be needed to review your physicians results, plan the surgery (including risk and benefits) and schedule.  Remember your surgery is a life changing experience and needs to be well thought out by you and planned by Dr. Johnson before proceeding to ensure the best results with the shortest recovery time.

Please assist us by providing the following information at the time of your consultation:

   
 
  • Your surgical referral slip and any X-rays if applicable
  • A list of medications you are presently taking and your Primary Care Physician Contact Information
  • If you have insurance, bring the necessary forms and card/information. This will save time and allow us to help you process any claims
 
   

IMPORTANT

All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.

For your surgery

   
 
  • Nothing to eat or drink 8 hours prior to surgery.
  • You will need an adult with a drivers license (18 years old or older) to accompany you to your appointment, stay in the office during your surgery and drive you home after surgery.
 
   

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, coumadin, Plavix, etc.)  These questions are addressed on the medical history form, please be complete as possible.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.  They may be emailed to X-ray@advancedfaces.com.

All Oral and Maxillofacial surgery patients will require a Panorex for proper diagnosis and treatment of any boney work needed.
